The N.C. Cooperative Extension of Person County office and the Person County Health Department are hoping to start another Diabetes Prevention Program (DPP) soon in 2021. We have a survey to determine if you are at risk for prediabetes. If you are interested in possibly participating in the next Diabetes Prevention Program, you will want to take a moment to complete this survey.

The program is not for anyone who has already been diagnosed with type 1 or type 2 diabetes. DPP focuses on anyone who might have pre-or borderline diabetes or who may be determined to be at-risk for diabetes (we can give you a simple risk test).
